Referral Process

A referral to hospice is appropriate when:

• Curative treatment is no longer the patient’s choice or option.
  
• Comfort care and symptom management become the primary focus.
  
• A patient has an life-limiting illness.
  

** Hospice services are available to patients without regard to disease, color, creed,national origin, age, sex, or ability to pay.

Referrals to hospice may be made by anyone, including but not limited to:

•   Family 
•   Nurses
•   Social Workers
•   Patients
•   Family Members
•   Nursing Homes
•   Home Health Agencies
•   Group Homes
•   Physicians

Who pays for Hospice care?

Both Medicare and Medicaid pay for 100% of the cost of care received from a licensed hospice provider.

Additionally, many private insurers provide a hospice care benefit.
